New CAT platform empowers Medicaid Non-Dual Members with resources and support, enhancing care for health plan participants
MobexHealth is proud to announce the launch of its groundbreaking platform focused on Cognitive Adaptation Training (CAT). This initiative is designed to empower Health Plan Care Management Teams to better serve Medicaid Non-Dual Members, particularly adults with disabilities and those aged 65 or older. These members often face complex healthcare challenges, including managing serious mental illness (SMI) and significant physical health needs.
Through the CAT Case Management and ER High-Risk Program, MobexHealth is helping members break down barriers to care, addressing not just clinical needs but also the Non-Medical Drivers of Health (NMDOH)—the social and environmental factors that impact health outcomes, such as the conditions where people live, learn, work, and play.
Targeted Rollout for High-Risk Members
The initial phase of the rollout focuses on members with a history of high emergency room utilization due to behavioral or physical health concerns. By leveraging the expertise of the Health Plan’s team of Community Health Workers (CHWs), these members have been located and engaged to receive vital support. This support includes connections to Case Management, Service Coordination, community resources, and other crucial services.
As part of the program, participating members will receive MobexHealth CAT Connect Tablets. These tablets are more than just a technological tool—they represent a customized, digital solution designed to meet the unique needs of each member. Through the CAT Connect platform, members can access valuable resources on the Health Plan’s website, as well as from trusted organizations that provide a variety of supports.
CAT Connect Tablets: A Lifeline for Members
The MobexHealth CAT Connect Tablets enable members to take greater control of their health by providing seamless access to the Health Plan’s member portal, telehealth services, and cognitive training games designed to improve mental acuity. The tablets also allow users to track their mood, locate healthcare providers, and connect to clinical care when needed via a direct link to the Health Plan Crisis Center and
Member Support Center.
With the push of a button, members can receive the assistance they need in real-time—whether it’s finding a provider, scheduling a telehealth appointment, or accessing life-saving resources during a mental health crisis.
Driving Healthier Outcomes
MobexHealth’s CAT initiative aims to improve health outcomes for Medicaid Non-Dual Members by reducing preventable emergency room visits, enhancing HEDIS compliance rates, and fostering better health education. By providing these members with the tools and resources they need, the platform will help them lead more independent, higher-quality lives. Additionally, the program is expected to significantly reduce per-member, per-month (PMPM) healthcare spending.
